Why Early Tooth Loss Requires Attention

Baby teeth aren't just for chewing — they hold space for the permanent teeth developing beneath them. When a baby tooth is lost prematurely (due to decay, injury, or extraction), the surrounding teeth can drift into the empty space, leaving insufficient room for the permanent tooth to erupt properly. A space maintainer prevents this from happening.

Types of Space Maintainers

Space maintainers can be fixed (cemented in place) or removable. Fixed maintainers are most common for younger children who may lose a removable device. The most common fixed type is a band-and-loop, which uses a metal band on an adjacent tooth with a loop that holds the space open. Bilateral maintainers (holding space on both sides) are also available for cases where multiple teeth are lost.

When a Space Maintainer Is Needed

Not every early tooth loss requires a space maintainer — timing and location matter. If the permanent tooth is very close to erupting, we may monitor rather than place a maintainer. We use X-rays to assess where the permanent tooth is and how much space needs to be preserved. An early evaluation is always the right call.

Caring for a Space Maintainer

Children with space maintainers should avoid sticky or hard foods that could dislodge or bend the appliance. Regular dental checkups allow us to monitor the maintainer and remove it once the permanent tooth is ready to erupt. Once removed, the space maintainer's job is done permanently.

Placement of a fixed space maintainer involves a band being fitted around an adjacent tooth and cemented. It's generally well-tolerated and takes just one short appointment.

How long does a space maintainer stay in?

Until the permanent tooth erupts into the space. This could be a few months or a few years depending on the child's age and development.
